Services
Assisted Living Services (AL)
Assisted Living is a housing option for people for who cannot or who choose not to live by themselves.
Our homes offer the opportunity for persons needing care to receive assistance in an intimate home setting. The smaller setting allows us to provide individualized care and attention to each of our residents as they need it. We offer professional service with a high ratio of staff-to-client assistance. Our services range from very basic assistance and reminders to memory care, “end-of-life care” and other more complex services as needed.
- Resident specific care plans
- Registered nurses on call 24 hours a day
- Part Time Nurse on site
- Nurse care coordination
- Nurse maintenance of clinical record information
- Nurse coordination of laboratory and physician requests
- Awake Home Health Aides on-site 24 hours per day
- 24 hour assistance with activities of daily living (ADLs)
- All personal cares including: bathing, dressing, grooming, etc.
- Incontinence management
- Medication management, storage and administration
- Housekeeping and maintenance
- Daily cueing and redirection as needed
- Laundry and linen services
- Nutritious fresh meals
- Regular exercise activities

Community Residential Settings (CRS) (Group Home)
Community Residential Settings are:
- Community participation
- Health, safety and wellness
- Household management
- Adaptive skills
- Interpersonal communications skills
- Leisure, recreation and socialization planning
- Skill-building to meet transportation needs
- Budgeting and assistance to manage money
- Cooking, meal-planning and nutrition
- Healthy lifestyle skills and practices
- Household chores, including minor household maintenance activities
